Patient Presents to GP and initial tests organised according to clinical indication

Referral to Community Cardiology Service (CCS)

Note: patients might be referred directly to the Hospital service depending on the clinical indication.

These may be passed to the Community Service depending on the Hospital triage decision for those patients.

Advice & Guidance to referring GP if appropriate

Triage by CCS Doctor

Referral to hospital cardiology service

Patient seen in appropriate CCS clinic(s):

1. Cardiology doctor

2. Echo

3. Ambulatory ECG/BP

All test results are reviewed by cardiology doctor

Further follow up appointment with CCS required?

Patient referred back to GP with outcome and plan
