Referral criteria
Service Referral Criteria
Inclusions:

Patients 18+ only. Under 18’s must be referred to appropriate hospital service.

Patients registered at a Mid Hampshire practice (click here for a full list) or at Fryern and Park/St Francis Surgeries in the Chandlers Ford area.
At present the service cannot see patients registered to a practice outside the practices listed above.
Services offered:
-
Electrocardiogram (ECG).
-
Transthoracic echocardiography (TTE).
-
Ambulatory ECG monitors (24-48hrs) and event recorders (7 day).

Ambulatory blood pressure recording (24 hr).
Conditions seen:
-
Suspected heart failure
-
Suspected valvular lesions/murmurs.
-
Atrial Fibrillation.
-
Palpitations.
-
Hypertension - diagnosis and management advice.
-
Known coronary artery disease - management advice.
-
Follow-up of valvular heart disease.
-
Follow-up of more complex cardiology cases in partnership with hospital team.
Conditions NOT seen:
-
Patients presenting with suspected unstable or acute cardiac presentations.

Patients with undiagnosed chest pain – these should be referred urgently to the hospital Rapid Access Chest Pain Clinic for assessment within 2 weeks.

Patients with complex cardiac conditions which may include implanted rhythm management devices e.g. CRT, ICDs.
The Community Cardiology Service cannot currently offer the following services:

Exercise stress testing.
-
Transoesophageal echocardiography.
-
Implantable ECG monitoring.
The service will not provide the following diagnostic procedures initially, although the range of investigations will be reviewed as the service develops in line with any changes to local guidelines.
